Prescription Assistance

The Prescription Assistance Program helps individuals to obtain a 3 month supply of their prescription medications from pharmaceutical companies. AmeriCorps volunteers located at Mid Delta Community Consortium (West Helena)  and working in the surrounding communities assists clients in submitting proper applications and correct paperwork to the pharmaceutical companies.

HOW DO I APPLY?

¨ Fill out the information form provide in this brochure.

¨ Bring  the information form along with proof of your income (copy of your payroll check, income tax return, proof of Social Security  benefits) to the Prescription Assistance Program (PAP) office.

¨ The Prescription Assistance Program coordinator will then review your information and locate any available programs through the pharmaceutical companies.

¨ Once a program has been located, you will have to pay a $5 fee for each prescription application. The fee is non-refundable.

¨ Complete the pharmaceutical company forms and have your doctor to sign them.

¨  After your doctor has signed the forms, you will need to have the forms mailed to the pharmaceutical company.

Once the pharmaceutical company receives your paperwork it usually takes approximately 4-8 weeks to process your paperwork and mail your medications
